You need to also fill out and print Medical information.doc and email it back to us:
Vaccinations needed:
- update your tetanus
- get typoid booster
- make sure you got your hepatitis B series (3 shots)
- if possible, get a BCG (for tuberculosis). AIDS is rampant and open TB has flared up
- start chloroquin or mefloquin for malaria
Important to bring along on the trip:
- US dollars (banks do not operate in Haiti currently)
- toiletries for 1 week (count on no showers!) (deo, toothpaste/brush, mouthwash, soap)
- food such as peanut-butter, dried fruits, dry-bread, nuts etc. for 1 week
- water can be bought in SD, don't bring from US
- personal flash-light (hand-held or on head)
- batteries for flash-light
- ciprofloxacin for diarrheal diseases
- get tetanus and typhoid vaccinations updated!
- start chloroquine (anti-malaria ASAP)
- bring anti-diarheal and anti-nausea for yourself (zofran is prescription; pepcid/peptobismol over the counter)
- wear sneakers or comfortable shoes
- if contact-lens wearer: bring your eye-glasses
- if on prescription medications: bring enough for 10 days in case of being stranded
- bring US phone which could switch to Haiti-SIM-card
- do not bring valuables at all
- don't bring camera's, we have full-time personal photo-journalist to document our trip and bring to outside world to inspire/inform others
- socks, t.shirts, scrubs
- face-masks and gloves if possible
